Evaluation Planning

Nuts and Bolts of Eval Planning

  • In general, reevaluations are federally mandated to occur every three years 
  • The reevaluation is meant to answer the following questions
    • Does the student continue to have a disability under IDEA?
    • Does the student continue to have a need for special education services?
    • What are the student present levels of academic achievement and functional performance?
    • Are any additions or modifications needed to the student's programming?

ISD 917 Evaluation Planning Timeline

Big numbers to remember:

  • Report must be DONE by (3 years minus 1 day) from the previous eval date
  • Hold eval planning meeting at least 15 days before you need to begin testing
  • Once signature is received, team has 30 school days to complete evaluation
  • Try to hold results meeting on Day 27, so there is time to edit
  • Final report must be postmarked on Day 30

First Steps:

  1. Complete Vision and Hearing Screening
  2. Review student records to inform your planning
  3. Connect with parent to determine if there are any outside evaluations that have been conducted and should be reviewed as part of the current evaluation plan 
  4. Offer an evaluation planning meeting and schedule the evaluation planning meeting 2 weeks before you want to start the evaluation

Hearing and Vision Screening

PRIOR TO starting the evaluation, vision and hearing MUST be screened by school nurses.

If no concerns are noted, then the evaluation may begin.

Failed Hearing Test

Nurse should follow up with parents about previous hearing tests, frequent ear infections, info from ENT, etc. If needed, IEP manager should contact their Lead Teacher or Assistant Principal to make a referral for 917 DHH team to take part in the evaluation.

Failed Vision Test

Nurse should follow up with parents about previous vision screenings, eyeglasses that might be at home, but not used, and future screenings. If needed, IEP manager should contact their Lead Teacher or Assistant Principal to make a referral for 917 teacher of blind/visually impaired team to take part in evaluation.

Evaluation Planning Meetings

It is best practice to hold an evaluation planning meeting that includes the evaluation team participants. The purpose of the meeting is to discuss all areas of concern, including any new areas or needs the student may be presenting, and develop the Evaluation Prior Written Notice (PWN).

  • The IEP Manager or School Psychologist facilitates the meeting

    • Discuss the reason for the evaluation

    • Invite parent thoughts/concerns

    • Guide discussion of each area to be assessed (each team member discusses their own part)

  • Designate a staff to take notes and/or complete PWN during the meeting

  • If PWN is complete, print and give a copy to parents for them to sign at the meeting

    • If parents sign, Day 1 is the next day

    • Finalize PWN in SpEd Forms after parents have signed

    • Upload the signed copy to SpEd Forms

  • Set date for evaluation results meeting if possible (around Day 27)

Evaluation Prior Written Notices (PWN)

Process for Writing the PWN

If a planning meeting is held:

  • School Psychologist completes boxes 1-5
  • Team members can add proposed Eval/Assessments to their relevant sections before the meeting
  • Designate a staff member to take notes and/or complete PWN during the meeting
    • Including adding/removing proposed Assessments
  • If PWN is complete, print and give a copy to parents for them to sign at meeting
    • If parents sign, Day 1 is the next day
      1. Finalize PWN in SpEd Forms after parents have signed
      2. Upload the signed copy to SpEd Forms
  • If parents do not sign, send a copy home with parents
  • If PWN is not complete at the meeting
    • Once the PWN is completed, a copy is sent to the parent or guardian for signature
    • Once the signed PWN is returned (or 14 calendar days have elapsed) the next day is day 1 for the evaluation
    • The IEP manager will email the team to inform them of the consent date and due dates for the evaluation
    • Site Admin Assistant emails IEP Manager and Psychologist with date mailed

If planning meet WAS NOT held:

  • School Psychologist completes boxes 1-5
  • Each team member adds their own proposed Eval/Assessments to the relevant sections:
    • IEP managers are responsible for making sure teammates get their parts in
  • Team members gather and document parent input in the Communication Log!
  • Site Admin Assistant emails IEP Manager and Psychologist with date mailed

ISD 917 Standard Practices

Standard Protocols

  • Sensory- hearing and/or vision screening completed by the Licensed School Nurse
  • Physical Status- health and medicine record review completed by the Licensed School Nurse
  • Assistive Technology- The Assistive Technology Checklist completed collectively by the team and summarized by the case manager
  • Other- The Developmental History and Family Home Questionnaire administered to parents by the case manager to gather “information reported by parent” 

When to Include a Functional Behavior Assessment (FBA)

  • FBAs are required for all initial evaluations when Emotional Behavior Disorder is a suspected area of disability.  
  • FBAs are required to develop a Positive Behavior Support Plan (PBSP)
  • Updating FBAs can be a required piece of the manifest determination process
  • The FBA should be updated if the student develops significant behaviors that are not addressed in their current FBA.
