Summary of Performance

The summary of performance is a written summary of a student’s most updated present levels related to academic achievement, functional performance, and post-secondary goals. The summary of performance provides information to other professionals who may work with your student beyond high school or transition programming. According to statute, it includes “recommendations on how to assist the child in meeting the child’s post-secondary goals” 34 C.F.R. § 300.305(e)(3).

  • The summary of performance should be completed as part of your student’s IEP meeting in either their last year of high school attendance OR their last year of special education eligibility. If the student's IEP occurs in the fall or winter of the student’s final year, the summary of performance will be completed and presented to parents as part of a spring transition meeting OR one month prior to the student aging out of special education services.

  • Beyond the student’s traditional IEP team members, it will be important to invite all outside service providers to this final meeting (i.e. Vocational Rehabilitation, County Social Worker, Social Security Case Manager, etc.). It is also recommended that the program social worker is invited to attend as a resource to county services that are not yet available. Your student should also be involved in the summary process, as they are able.  

    If the summary of performance is being reviewed as part of a spring transition meeting, team membership should include those team members involved in transition planning: case manager, parent/guardian, student, county case manager, vocational rehabilitation, school social worker, etc.

  • Address the following in each section:

    • Academic Achievement: Summarize your student’s current progress toward academic goals.
    • Functional Performance: Summarize your student’s current progress toward functional goals.
    • Post-secondary recommendations, address the following for each section:
      • current transition goal(s)
      • progress toward transition goal(s)
      • current work/volunteer experience
      • current supports/services provided by outside agencies
      • list of recommended accommodations/modifications/assistive technology (identify from the IEP which of these will support the student’s success with post-secondary goals).
  • The Summary of Performance will be shared with team members as part of the students IEP or spring transition meeting. This document should also be finalized in SpEd Forms and filed in the cumulative record. A copy should be sent home to parent/guardian(s). 

Steps for completing the SOP

At either the final IEP meeting (may be an amendment at the end of the school year) we will do the following:

  1. Amend the IEP (if necessary)
  2. Update the anticipated date of graduation on the transition page
  3. Update the LRE Statement
  4. End Service Minutes or update the end dates

MM/DD/YYYY: CHLD's IEP Team met so that they could present their Graduation Portfolio which is a requirement of their IEP-Driven Diploma Plan, to review his Summary of Performance as they have met the stipulations of their IEP-Driven Diploma Plan for graduation, and discontinuation of special education services due to high school graduation. The district updated their anticipated graduation date that can be found on the transitions page of this IEP, as well as ending their service minutes to reflect an end of special education services as they have met the requirements set by (RESIDENT DISTRICT) and ISD 917 to graduate and receive their high school diploma.

  • Following the IEP Team Meeting held on MM/DD/YYYY for CHLD to present their graduation portfolio, which was a requirement of his IEP-Driven Diploma Plan, discuss their Summary of Performance, and the discontinuation of special education services due to high school graduation, the district is proposing the following amendments:

    The district is proposing to update CHLD's anticipated graduation date found on the transitions page of

    CHLD's current IEP to reflect the change of their high school graduation date.

    The district is proposing to end special education service minutes for CHLD as they have met the district requirements for graduation, and special education services will be discontinued as of XX/XX/XXXX

    The district is proposing to update CHLD's LRE to reflect the change in special education placement.

  • The district is proposing that special education requirements and the IEP end when CHLD  graduates; therefore, services will no longer be provided. CHLD was on an IEP-Driven Diploma Plan where they were required to complete and present their graduation portfolio that consisted of the following: 

    Make progress on their IEP goals 

    Continue to meet with a VRS Counselor 

    Complete a transition portfolio that includes tasks related to employment, post-secondary education, and independent living 

    Obtain important documents such as a state ID, SS card, and birth certificate 

    Complete Exit Presentation to IEP Team & important guests 

    Obtain 6 months of employment from the same business/company

    Team review of present levels of performance

  • The team considered postponing graduation until the end of the school year in order to meet CHLD 's transition needs in the areas of employment, post-secondary education, and independent living, however this was decided against as they created a graduation portfolio that met the criteria for these areas.

  • Special education services will be discontinued as of MM/DD/YYYY. CHLD  has been informed that all special education services will be ending on this date.

EXAMPLE  

According to daily classroom performance checklists,  Johnny completes his assignments on time, with 70% accuracy or greater, 65% of the time. His teachers report that Johnny is most consistent at completing math assignments, with an overall on-time completion percentage of 87%, and reading assignments are most difficult for Johnny, with a rate of 51%. The team will continue to work on building his skills and finding motivating strategies, particularly in the area of reading.

Johnny has maintained his independent participation in academic, recreation and leisure, and functional vocational activities from an average level of 80% to an average level of 95%. Johnny has met this goal and will continue working on this goal for maintenance and generalization of his skills.

Post Secondary Education & Training

Johnny has completed interest inventories to identify his interests and future career goals. 

As a CHLD at (SCHOOL), Johnny’s team has learned that his disability impacts his ability to attend to instruction, maintain attention toward tasks, engage in academic tasks/activities, thereby impacting his grades, test performance/accuracy, and overall level of progress in the general education curriculum.

Johnny has the following accommodations, modifications, and supports that can be used in a future post-secondary setting to assist in learning: 

  • Additional time to complete testing and classroom assignments.
  • Instructions will be repeated and restated, and tests in subjects other than reading will be read to him.
  • Frequent check-ins by staff to ensure understanding of directions.
  • The opportunity to work in a small setting with as few distractions as possible to optimize learning and the ability to complete tasks.

Independent Living

  • Insurance information.

  • Medications, if needed (type of medication, how often does it get refilled, where do you get them).
